How to set fields as protected health information?

Google Forms does not support PHI fields, but you can use Formfacade - Prefill & email addon to mark the fields as protected health information.


How to mark fields as protected health information in Google Forms?


  • Open your form in Google Forms

  • Click on the addon icon

  • Click Formfacade - Prefill & email

  • Click Create prefill links

  • Click Proceed

  • Share page will open in a new tab

  • Click Edit

  • In the Edit page, click on the gear icon next to the field

  • Field settings page will be displayed

  • Mark the field as Protected Health Information

  • Click Save
